Responsibilites: Direct Patient Care- Provide targeted, evidence-based assessments and interventions in brief 30-minute appointments for a wide range of behavioral, chronic medical, and health conditions.Team Collaboration-Act as a core member of the Primary Care team. Participate in daily huddles, facilitate "warm handoffs" from providers, and offer expert consultation on patient care.Program Development- Develop, teach, and oversee health education classes for patients. Provide training to primary care staff to enhance the integration of behavioral health services.Clinical Excellence-Utilize a stepped-care approach to maximize patient access. Monitor treatment effectiveness with measurable outcomes and ensure timely, concise documentation.
